As a cosmetic urologist, I think it’s important to bring clarity to this conversation to help patients achieve the precise look that they desire and understand the benefits and risks of each style.

This terminology refers to where the circumcision scar appears along the shaft.
Anatomically speaking, what patients call “high” often refers to a scar placed closer to the body (more proximal), while “low” refers to a scar closer to the glans (more distal). This seems backward when we think in terms of anatomy, which is why I prefer to describe it more precisely.
Our Approach:
This part of the style equation refers to how much shaft skin remains after circumcision. This outcome is determined primarily by how far apart the two incisions are placed.
Loose Circumcision:

While popular, a tight style must be approached carefully. An overly tight circumcision can create tension on the shaft, pull scrotal skin onto the base of the penis (creating a penoscrotal web), and shorten the visible length of the penis.
