Pearly penile papules, or PPP, represent a common source of concern. These are benign, non-infectious growths that typically appear along the coronal margin—the ridge just behind the head (glans) of the penis.

PPP are:

• Benign angiofibormas that appear as small, dome-shaped growths or short, finger-like projections
• More common in uncircumcised men, but can occur in anyone
• Often mistaken for genital warts or other STIs, despite being entirely harmless

Why They Matter

Although PPP are clinically insignificant, their impact can be very real:

• Anxiety or embarrassment in intimate settings
• Concerns from partners who mistake them for something infectious
• Negative self-image or hesitation during sexual activity

Unfortunately, many patients are told that because PPP are benign, they should just live with them—often without any meaningful discussion of treatment options.

While not medically necessary, treatment for PPP is absolutely reasonable when the patient feels they negatively affect confidence, sexual function, or aesthetic appearance.

Treatment Options Have Included:

• Laser ablation

Our Preferred Approach: Precise Treatment with Minimal Scarring

Through extensive experience treating PPP, we've found the most effective and cosmetically pleasing method to be a hybrid technique:

1. Fine surgical excision of each papule
2. Followed by highly focused electrocautery to the base of each lesion

This two-step approach allows for:

• A flat, smooth surface post-treatment
• Minimal risk of scarring
• Only mild hypopigmentation (skin lightening)
