Can I Lengthen My Penis?

We receive many calls daily from patients asking “can I make my penis longer?”

The answer really depends on a guy’s specific goals. While men looking for increased flaccid length and overall increased erect size are excellent candidates for the Himplant, a different approach is best for guys looking for reliable increases in erectile length.

The One Real Option for Erect Lengthening

Removing suprapubic fat is the single most effective and reliable way to increase effective erect penile length. This is also the most overlooked and most underestimated approach.

Perceived Penile Shrinkage and Suprapubic Fat

Many men feel that their penis has “shrunk” over the years. Countless patients have come to clinic reporting loss of 2-3 inches from their glory days in their early 20s. While true penile shrinkage may occur due to atrophy of the erectile tissues in the setting of certain post-operative states (especially patients with prior pelvic surgery), most men reporting penile shrinkage have simply lost access to a portion of their penis.

Over time, most men accumulate fat just above the penis in an area known as the suprapubic region. This resulting fat pad doesn’t affect the penis itself, but rather conceals a portion of the shaft, limiting the visible and accessible length.

Patients almost always have a skeptical look on their faces when I tell them about the inches of penis they have locked away in suprapubic fat. I find that their skepticism quickly melts away when they see a CT image of the suprapubic region from a sagittal or “side” view.

The following image is a side (or sagittal) view of a man’s abdomen and pelvis. For the sake of orientation, his abdomen is facing the left side of the screen and his back is facing the right side of the screen. I’ve marked the distance between his skin and the pubic bone showing a 3.28 cm (1.3 inch) deep fat pad. This is pretty modest and typical for a trim middle-aged man.

The next 3 images demonstrate men with increasingly large fat pads ranging from 4.26 cm (1.7 inches) in image 2 to 8.78 cm (3.5 inches) in images 4. A loss of 3.5 inches isn’t subtle. It’s a massive change in perceived length in a guy who otherwise would likely be well above average.

How We Treat Excess Suprapubic Fat

There are two approaches we use, depending on the patient’s anatomy and goals:

1. Suprapubic Liposuction

  • Minimally invasive
  • Removes localized fat above the penis
  • Results in improved visibility and projection of the penis

2. Suprapubic Fat Pad Excision

  • More aggressive, surgical approach
  • Allows for removal of deeper or more adherent tissue
  • Particularly effective for men with significant fat accumulation or skin redundancy

In both cases, what we’re doing is unlocking the penis that’s already there—not adding anything artificial, not altering anatomy, just revealing more of what was hidden.

Why Patients Are Skeptical

It might sound too simple. It might sound like a cosmetic trick. But for many men, this procedure offers the most dramatic, measurable improvement in visible length, both flaccid and erect.

And unlike questionable techniques like suspensory ligament incision (which often fails or scars back tighter), suprapubic fat removal works—and it works predictably.

Bottom Line

If you’re serious about increasing visible erect length, stop chasing gimmicks and start with anatomy. Removing suprapubic fat—whether through liposuction or excision—is the gold standard for real, lasting improvement in penile projection and length.
